The aim of the project is to assess the feasibility and impact of an electronic, self-assessment among end stage renal disease (ESRD) patients. In addition, they will explore associations between symptom scores and clinical outcomes such as Quality of Life, Emergency department visitation, hospital admission, functional status, and mortality among a diverse sample of ESRD patients receiving dialysis, with particular attention to minority and elderly patients.
