Project Period
The overall objective of this proposed study is to adapt and co-design the Inpatient Hospice Transitions Checklist (I-HTC). The I-HTC is intended to support clinician-led conversations about hospice eligibility, hospice care transitions and decision-making, and be embedded within an existing EHR ACP note to allow real-time access and documentation during patient encounters. The overarching goal is to improve the quality, consistency, and timeliness of hospice-related communication and surrogate understanding of hospice care.
