Health Clearance Requirements Frequently Asked Questions

To assist you in completing your health clearance paperwork, below are some of the most frequently asked questions regarding health clearance requirements. We have also included a few tips and video tutorials for incoming students to help streamline your process.

FAQ's

Yes. Due to our contracts with clinical partners, you must have a full dose series (one dose of the Johnson & Johnson vaccine, or two doses of the Moderna/Pfizer vaccine or the Bivalent vaccine.

No. If you have a negative Measles, Mumps, Rubella (MMR) test and/or Varicella titer, you do not need to get revaccinated as long as you can provide proof of childhood immunization history. Please note we do not accept booster shots.

If you have a negative Hepatitis B titer and received the Hepatitis B 3-series vaccinations, you will need to fill out Form D and either decide if you are going to decline the vaccine or get revaccinated with the Hepatitis B 3-series vaccinations.

The required vaccines for all students are the MMR, Varicella, Hepatitis B, Covid-19, and Flu vaccines.

The health clearance form that needs to be redone annually is the medical flu vaccine (Form C).

Only a licensed healthcare provider can fill out the health clearance forms. You cannot fill out the forms yourself. You can go to your primary care provider, the NYU Student Health Center (SHC), or any urgent care to get the forms filled out.

N-95 fit testing results are only valid for one year. Fit testing is mandatory and is to be renewed annually. For undergraduate students, fit testing must be done at NYU Meyers. For graduate students, fit testing can be done at your place of employment, NYU Meyers, or any certified facility.

You must provide forms A-1, A-2, B-1, B-2, Form C, a copy of your titers lab test results, tuberculosis lab test results, Covid 19 documentation. Lab test result documents must have demographic information such as your name, DOB, the name of the doctor that ordered the test, when it was collected, when it was read, etc. For graduate students, the Office of Clinical Affairs will send you a correspondence email regarding additional required documents.

For graduate students, you must send the required health clearance documents scanned as a PDF via email at nursing.healthclearance@nyu.edu

For undergraduate students, you must upload your required health clearance documents to your American DataBank/Complio account.

If you have copies of your childhood immunization history records and previous titers lab test results, they can be used to fill out forms B-1 and B-2 as long as they are no older than 10 years. If your forms B-1 and B-2 have been completed in one of our programs in the past, they can be resubmitted. The annual forms will need to be renewed.

We require forms- B-1 and B-2 to be completed and submitted. Even if you have your childhood immunization history records, you will still need to provide copies of your titers lab test results as well.

The drug screening can be done outside of New York State. However, it cannot be done outside of the United States. The drug screening must be done through American DataBank/Complio.

Go into your American Data Bank/Complio account, navigate to the CPR area, and then select the "Date of NYU CPR Course" after registering for the NYU Meyers BLS Certification Course via NYSIM. Enter the date of your NYSIM in-person skills course to be temporarily compliant (for about 30 days) until the certificate is issued. It is okay if the date of the BLS course is added after the health clearance deadline if you are doing it through NYSIM.

New Sequence 1 undergraduate students are required to complete their Basic Life Saving (BLS) certification at NYSIM. Graduate students can use the American Heart Association website to locate a BLS training program in their area.

For graduate students, fit-testing can be done at your place of employment, NYU Meyers, or any certified facility. Your face must be clean-shaven for your fit-testing.

Upon registering for the online course, learners receive a direct link via email to be activated and saved to their AHA e-learning account.

The Heartcode BLS online course completion certificate must be handed to the front desk upon sign-in for the skills check.

You will be practicing skills that require working on your hands and knees, standing and bending, so wear loose comfortable clothing.

Nursing students must have on shoes that are: white, closed-toe, closed-back, leather-like finish and non-porous (i.e. cannot be mesh, without a back, or clogs). Here is a shoe guide for your reference.

Additional tips

  • Please make sure to obtain your NYU Student ID and activate your nyu.edu email as soon as you receive your welcoming email from the Nursing Advising Office.
  • For incoming new nursing sequence students, schedule an appointment with your healthcare provider to complete your health clearance forms as soon as possible after you receive the welcoming email from the Nursing Advising Office and Office of Clinical Affairs.
  • Make sure your health clearance forms are completed in their entirety before leaving your healthcare provider’s facility.
  • Please note that the health clearance requirements for the NYU Meyers Office of Clinical Affairs and the NYU Student Health Center (SHC) are different. You must complete them both.
