The specific aim of this project is to gather input from key HHC stakeholders on the types of essential patient information for meaning communication and use-friendly design features of a future mobile application, intended for use by HHC providers and family caregivers of Chinese Americans living with dementia, that will support timely patient condition monitoring and early clinical intervention for emerging medical issues.
