The objective of this megagrant application is to refine and pilot a culturally sensitive health and fitness lifestyle intervention, TeamHEALTH, which will serve Black adults who have been prescribed diet and exercise to manage or prevent a chronic condition. TeamHEALTH aims to address health disparities in chronic conditions on multiple socio-ecological levels, including individual, interpersonal, and community, through coaching (i.e., physical activity, diet, and exercise), community engagement, and social support.
