Clinical Session Tracking Form Faculty FAQ

  • Go to https://nursing.nyu.edu/forms/clinical-session-tracking
  • Authenticate with your NYU ID and password.
  • On the top of the page, you should see a menu bar. Click on structure, then click on webforms.
  • Under "Filter Webforms," type "Clinical Session Tracking" and press enter. You should see the tracking form as a result ("Webform form: Clinical Session Tracking").
  • Click on the Results button next to the correct form.
  • On the next page, click on "Results" from the menu tabs. This will take you to a chart with ALL of the current student submissions.

 

  • The top portion of the page can help narrow down results to what you need
  • The "Search" bar allows you to search using any data that might be located in the chart below (e.g student name, course title).
  • The "Request Type/Status" allows you to filter results by Open Sim appointments, Absence requests, modified, attended, and canceled requests. You can choose multiple
  •  The location filter will narrow results down to either on-campus or off-campus results.
  • You can filter down results to date by using one of the date filter sections. Please make sure to use the appropriate date field to generate the correct results. For example, only use the "Off-Campus Date/Time" to search for dates that only relate to an Off-Campus Clinical Makeup
    • Avoid using the Search bar for any date-related information, as it may leave out some of the criteria.
  • There are Additional Filters that allow you to search by Semester, Course Name, and Course Coordinator
  • Once you have made all your filter selections, make sure to click on "Apply" to see the results.

  • Once you have filtered the data you need from the chart, make sure that under the "Action" drop-down, Star/Flag submission is selected.
  • Click on the box next to "Download Flag" to select ALL of the result options.
  • Scroll to the bottom of the page and click on "Apply to selected items."
  • Once you receive confirmation (under the "Download Flag" column, all your selections should be filled in with "Yes"), scroll back up to the top of the page and click on Download.
  • Scroll down to "Column Options" and select the columns you would like to see. De-select any columns you do not need in your downloaded file.
  • Scroll down to "Download Options." Make sure to click on the "Starred/flagged submissions" box is checked. 
  • Then click on download. You will have a new excel download with the information you flagged.
  • Go back to the Submissions page, and scroll down to the "action" section again. Select Unstar/Unflag submission.
  • Click the box next to "Download Flag" to select ALL of the options
  • Scroll down and select "Apply to selected items." This process will allow other users to download the information they need without also seeing the data you downloaded. 

  • The flagged items will remain chosen for download, even after the user is finished. Therefore you must unflag all the selections before downloading any data.
  • Before filtering out any data, scroll down and click the box next to "Download Flag" to select ALL of the options.
  • Under the "Action" drop-down, select Unstar/Unflag submission.
  • Click on "Apply to selected items." All of the previous flagged options will be unselected.
  • You may now begin the process of filtering and downloading the information you need. 

  • Students will need to report that they attended and completed their makeup session by inputting this code AFTER attending their clinical makeup session. Do NOT give the students these codes. The instructor leading their makeup session will give them the code after their makeup.
  • If a student is absent because of mandatory COVID quarantine, they will be assigned an asynchronous assignment. Once the assignment is completed to your satisfaction, you can provide student with the code so that they can report their makeup as completed.
