Project Period
The purpose of this study is to optimize the An Sante ak Dyabét (ASAD) “Let’s Live Healthy with Diabetes” Self-Management Education (SME) Program and ensure its seamless integration into the workflow of each Federally Qualified Health Center (FQHC). This will be achieved by determining specific implementation strategies, including defining roles, responsibilities, and necessary training that are tailored to the needs of each clinic/health center.
