The long-term goal of this project is to generate findings to support the development of scalable interventions to address the palliative care (PC) needs of diverse residents living in long-term care. This work will lay the foundation in which to conduct intervention-based research in Medicaid-funded assisted living (M-ALF) and provide important lessons in PC communication in which to build PC interventions for diverse populations supported by Medicaid. This approach may also inform PC interventions in well-resourced standard Assisted Living Facilities (ALFs) as well as under-resourced M-ALFs. More importantly, after establishing the feasibility of using community-health workers (CHW) to address PC needs in this setting, we will disseminate the findings for feedback with state-level partners (Leading Age, Health and Housing Consortium) with the aim of integrating the process into admissions procedures and 6- month care plan review for all NY state M-ALFs using policy as a driver.
